Travel Vaccination

Our nurses can provide travel health advice and arrange the appropriate immunisations. Please download and complete the travel form, then send it to Y00412.bhallas@nhs.net minimum 6 week's before your travel date. Vaccines need time to take effect, and some may require a course spread over several weeks.

Travel Vaccine Appointments

Appointments are subject to availability. If we are unable to accommodate your request, we will advise you to visit a pharmacy who can provide travel vaccinations.

Pharmacy Notes:

Pharmacies will charge for all travel vaccines. Please enquire directly with your chosen pharmacy about costs.

GP Practice:

We will issue the prescription, which you will pay for the prescription.

Once the prescription has been collected, we will book an appointment with the nurse, who will administer the travel vaccines free of charge.

Please Note:

We do not provide Yellow Fever vaccinations.

We do not prescribe malaria medication. For both services, please enquire with the pharmacies who provide travel vaccination.

Change of Address

Change of Address 

If you have recently moved home, please complete the Change of Address form below to keep your details up to date, so we can maintain accurate medical records

  1. Click here to download the Change of Address Form.
  2. Print and complete the form with your updated details.
  3. Return the form by emailing it to Y00412.BHALLAS@nhs.net or Bringing a printed copy to the reception desk.

In some cases, we may ask patients to provide proof of address, such as a utility bill, council tax statement, or rental agreement.

Please check all sections of the form are completed clearly. Failure to provide full details may delay the update of your records.

Note:  Before submitting the form, please check the practice catchment area criteria. If your new address is outside the catchment area, we recommend registering with a practice nearby to your home, as we are unable to provide lot of services once you are out of our area.

MEDICAL RECORDS- REQUEST

Access to Health Records (Subject Access Request)

To request access to your medical records, please complete the Medical Records Request Form.
You may download the form Click here to download the Form and either email it to us Y00412.bhallas@nhs.net or bring the completed form to the surgery and hand it in at reception.

Processing Time
In line with Data Protection and General Data Protection Regulation (GDPR) guidelines, medical record requests can take up to 28 days to process.
Once your form has been received, we will prepare your records and contact you when they are ready for collection.

Before You Submit a Request
Please allow the full 28-day processing period and submit your request at least 28 days before you need your records. This ensures we can prepare your information accurately and securely.

Collecting Your Records
When attending to collect your records, please arrive during our opening hours. You may be asked to provide proof of identity to ensure we release the records to the correct person.
If someone is collecting records on your behalf, written consent and identification will be required.

If we need any additional information to complete your request, a member of our team will contact you.
